ABSTRACT

Portal vein thrombosis is one of the common complications of liver cirrhosis, and anticoagulation is currently the main treatment method for this disease. Since low-molecular-weight heparin must be injected and vitamin K antagonists require regular monitoring of international normalized ratio (INR), direct oral anticoagulants have become a research hotspot in replacement therapy with the advantages of convenient oral administration, no need for INR monitoring, and high recanalization rate. This article summarizes the advances in direct oral anticoagulants in the treatment of cirrhosis-associated portal vein thrombosis, in order to lay a foundation for further clinical studies.

ABSTRACT

【Objective】 To construct the secretory expression system of insect cells to express the secretory TSHR A subunit protein in the ovarian cells of Spotoma oryzae (sf9). 【Methods】 A recombinant plasmid containing the target protein was constructed, and then the positive bacmid was screened out by the blue and white spots experiment. The verified bacmid was transfected into SF9 insect cells to obtain recombinant baculovirus. The virus was amplified, and the titer level was detected by virus plaque assay. Finally, Western blotting was used to identify the expression of the recombinant protein and optimize the expression conditions. 【Results】 During the construction of the protein expression system, PCR identification and sequencing results confirmed the correctness of the sequences of the recombinant plasmid and the recombinant bacmid. After the transfection of the bacmid, the signs of virus budding were observed in sf9 cells. The virus was collected and amplified. The titer of P1 generation virus was 2×107 pfu/m according to the plaque assay. The recombinant protein was identified by Western blotting and confirmed to be exogenous into the culture medium. The optimal condition for virus infection and protein expression was 72 h after the infection when the multiplicity of infection (MOI) was 1. 【Conclusion】 We constructed an insect cell expression system secreting TSHR 22-289 (55 ku), and the protein could be successfully glycolyzed. This system provides a preliminary basis for the construction and production of its industrial platform and also provides a useful tool for studies on TSHR protein and prevention of GO in the future.

ABSTRACT

Objective:To explore the application effect of online teaching mode on nursing experimental course. Methods:A total of 615 students in 4 classes of the Batch 2017 nursing undergraduates completed learning tasks such as teaching videos, notifications and tests on the ChaoXing platform (hereinafter as "ChaoXing"). And they learned online and had interactive discussions through Zoom cloud meeting system (hereinafter as "Zoom"). Online questionnaires were conducted to collect feedback on classroom teaching effects. SPSS 25.0 was performed for statistical analysis. Results:Among all nursing students, 445 (76.72%) students rated the content of the experimental course with a score of 16 or above, 540 (93.11%) ones felt that the "course content integrity" was good, 439 (75.70%) ones felt that "they have enough time to understand the important and difficult knowledge", 432 (74.48%) ones thought that "this learning method is very interesting", and 181 (31.21%) ones believed that "compared to the traditional way, this kind of teaching method is preferred" and thought online teaching could effectively enhance the enthusiasm for learning at home. While, 185 (31.90%) students disagreed with those above because that the instability of the network affected the sense of class experience, the lack of molds at home and the simulated clinical environment affected the sense of participation, and the inconvenience of taking notes or without books, etc. Conclusion:The "ChaoXing + Zoom" teaching method has been applied to nursing experimental teaching and achieved good results, but it still needs to assist offline practice to consolidate operational knowledge and skills. This teaching method is worthy of popularization and application during the epidemic period.

ABSTRACT

Objective:To investigate the status of time management and self-regulated learning of undergraduate nursing students, and to explore the correlation and influencing factors between them. Methods:In March, 2020, 873 undergraduate nursing students from a nursing college of a medical university in Chongqing were investigated with General Information Questionnaire, Adolescent Time Management Disposition Questionnaire and Self-regulated Learning Status Questionnaire. SPSS 22.0 was used for data analysis. Independent sample t-test, one-way ANOVA, pearson correlation analysis and multiple stepwise regression analysis were used for further analysis. Results:Total scores of time management disposition and self-regulated learning status were separately (153.58±18.62) points and (81.73±14.11) points. Scores of both items were influenced by different grades, whether or not they were student cadres, whether they voluntarily applied for nursing profession and whether they would like to take the postgraduate entrance examination. Correlation study showed that the score of time management disposition was significantly positively correlated with the score of self-regulated learning ( P < 0.01). And multiple stepwise regression study showed that time value, time monitoring and time efficacy entered the regression equation ( R2 = 0.353, adjusted R2 = 0.351, F = 153.073, P < 0.01). Conclusion:The undergraduate nursing students' time management disposition and self-regulated learning are at a moderate to good level. Each dimension of the time management disposition somewhat forecasts the self-regulated learning learning situation of the students. The self-regulated learning status of the nursing students can be enhanced by fostering their time management abilities.

ABSTRACT

Objective:To study the influencing factors of diabetic retinopathy (DR) in patients with type 2 diabetes mellitus (T2DM), and to explore the control countermeasures. Methods:The patients definitely diagnosed with T2DM in the Ophthalmology department from July 2017 to January 2019 were enrolled as subjects. The clinical data of patients were collected. According to eye examination results, they were divided into T2DM with DR group and T2DM without DR group. Clinical data of both groups were compared. Multivariate stepwise logistic regression analysis was performed to analyze the risk factors for DR in T2DM patients. Results:The proportion of diabetic nephropathy and diabetic peripheral neuropathy in T2DM with DR group was significantly higher than that in patients with T2DM without DR ( P<0.05). The T2DM duration, retinal arteriosclerosis, diastolic blood pressure (DBP), systolic blood pressure (SBP), glycosylated hemoglobin (HbA1c) and low-density lipoprotein cholesterol (LDL-C) in T2DM with DR group were significantly higher than those in T2DM without DR group ( P<0.05). The postprandial 2h C-peptide in T2DM with DR group was significantly lower than that in T2DM without DR group ( P<0.05). Multivariate stepwise logistic regression analysis showed that diabetic nephropathy , diabetic peripheral neuropathy, T2DM duration, SBP, DBP, HbA1c and LDL-C were the influencing factor of DR. Conclusions:Diabetic nephropathy , diabetic peripheral neuropathy, T2DM duration, SBP, DBP, HbA1c and LDL-C are related to occurrence of DR. In the control, health education should be strengthened for high-risk populations with long T2DM duration. In the community management, the management for blood lipids, blood pressure and blood glucose in diabetic patients should be further strengthened.

ABSTRACT

@#The access negotiations of National Reimbursement Drug List (NRDL) in China from 2017 to 2019 effectively improved the availability and affordability of high-value innovative drugs. However, the actual outpatient reimbursement of most negotiated drugs is lower, which affects the patients" health insurance funds. In this paper,97 negotiated drugs in 337 overall planning cities were selected as samples to analyze the outpatient reimbursement for negotiated drugs. The results showed that the reimbursement level of 40 negotiated drugs was less than 50% in over 70% cities, which can be mainly interpreted as absence or imperfection of outpatient reimbursement policies for special diseases. Finally, this paper puts forward suggestions and protocols for improving the outpatient medical insurance through realizing outpatient overall planning, improving outpatient reimbursement policies for special diseases, and exploring innovative payment, ensuring the achievement of the access to NRDL.

EAGD mice were induced with an adenovirus expressing the human thyroid stimulating hormone receptor antibody A-subunit (Ad-TSHR289). DHT (5mg) or a matching placebo were implanted one week before the first immunization. Thyroid hormones were detected with radioimmunoassay kit.. Cytokines [such as interferonγ (IFNγ), interleukin (IL)-4, IL-10, IL-9, and IL-17] producing cells from the spleen were detected using flow cytometry.@*Results@#As expected Ad-TSHR289 treatment increased total thyroxine [EAGD group vs. control group: (117.76±32.69) nmol/L vs. (33.08±12.61) nmol/L, P<0.0001] and free thyroxine [EAGD group vs. control group: (15.01±11.55) pmol/L vs. (3.55±1.88) pmol/L, P<0.0001]. Treatment of DHT slightly lowered thyroid hormones [DHT group vs. placebo group: total thyroxine (114.80±44.27) nmol/L vs. (123.17±77.73) nmol/L; free thyroxine (13.48±6.01) pmol/L vs. (14.19±12.65) pmol/L], without significant difference (all P>0.05)]. However, the percentage of IL-10, but not IFN γ, IL-4, IL-9 and IL-17, secreted spleen cells increased in DHT group than in the placebo group [(7.11±3.29)% vs. (3.51±1.36)%, P<0.05].@*Conclusion@#The effects of DHT on thyroid hormone are mild. It might play an immunomodulatory role in the male mouse Graves disease model by up-regulating the cytokine IL-10.

ABSTRACT

Objective:To establish the characteristic T cell receptor(TCR) repertoires which could distinguish the Graves′ disease(GD) and thyroid associated ophthalmopathy(TAO) immune repertoire for precision diagnosis. Methods:Fifteen Graves′ disease patients(nine patients without Graves′ ophthalmopathy and six with Graves′ ophthalmopathy)were enrolled in this study, and they were diagnosed less than 2 months and were naive to treatment. Twenty healthy volunteers who were age and sex matched served as controls. The blood samples were collected at diagnosed. RNA was isolated for TCR repertoire sequencing. Results:TCR diversity in GD and TAO patients was significantly lower than that of healthy controls. With TCR high-throughput sequencing data, characteristic TCR databases of GD and TAO, containing 157 and 116 VJ families, was constructed. In these VJ families, both common and unique families of the two diseases existed. Conclusion:GD and TAO have common pathogenesis, but have different dominant T cell clones, which mediate different immune responses. Therefore, the characteristic TCR of GD and TAO patients may be used as a potential molecular marker.

ABSTRACT

Objective To survey the status of aspirin application asautiplatelet therapy for secondary prevention of coronary artery disease (CAD) in Beijing communities. Methods A cross-sectional survey on status of aspirin use was conducted among 60 communities in Beijing from September 2014 to January 2015.Items of survey included the duration of aspirin application, aspirin dosage, application time, reasons for interruption and reasons for non-application.Results Total 61 000 questionnaires were delivered and 56 969 valid ones were retrieved with a recovery of 93.39%.The aspirin application rate was 78.22% (15 105/19 311)among patients with diagnosed CAD,while that was 81.42% (1 319/1 620) among patients with self-reported CAD (x2=9.06,P<0.01).There was no effect of early cardiovascular disease family history on aspirin application among patients with diagnosed CAD (P =0.77).Among the patients with diagnosed CAD,the reasons of no aspirin application were:rejection with unknown reasons, worry about stomach side effects, clopidogrel application, not suitable subjectsand other antiplatelet drugs application. Conclusions The proportion of aspirin application is relatively high among coronary artery disease patients in Beijing communities. The results of survey provide the direction of patient education in the future.
